We accept Medicare referrals under the GP Chronic Condition Management Plan (GPCCMP) for eligible clients with chronic medical conditions.
To access Medicare rebates, you will need a referral from your GP as part of a multidisciplinary care plan. A chronic condition is one that has been present for six months or more, such as:
Osteoarthritis, Rheumatoid Arthritis, De Quervain’s Tenosynovitis, Trigger Finger/Thumb, Tennis Elbow, Golfer’s Elbow, or Stroke.
Eligible clients may receive up to five subsidised allied health sessions per calendar year, with a rebate of $61.80 per session.
